This photograph is from 1913, city of Kostroma.
Alexis was carried in public processions after his leg injury and near death in late 1912 at the hunting lodge at Spala in Poland.
The book, The Last Tsar (1996) by Larissa Yermilova has the same photo on page 231.
The caption says that the photo was taken in Kostroma at the St. Hipatius Monastery in 1913, which went down in history as the cradle of the Romanov dynasty.
That was the only time that he needed to be carried in public.
